What Is Tendinopathy?

Tendinopathy is a change in a tendon's structure and capacity, usually from more load than it's currently able to handle — not a simple “-itis” inflammation. It shows up as localized pain, stiffness, and reduced tolerance for the activities that once felt easy. Pain intensity doesn't always match how much tissue change is present, so a painful tendon isn't necessarily a badly damaged one.

Why Rest Alone Doesn't Fix It

Tendons adapt to the load placed on them — too little, and they lose capacity; too much, too fast, and they get irritated. Complete rest often makes a tendon less prepared for the activity you want to return to. The fix isn't less load — it's the right load, applied and progressed on purpose.

  2. Build strength — progressive, heavier resistance training that rebuilds the tendon's capacity over several weeks.

Research on tendinopathy consistently supports progressive loading:

  • Structured loading programs outperform rest and passive treatments for pain and function

  • Heavy, slow resistance training measurably builds tendon capacity

  • Using next-day pain as a guide — not fear — helps set the right amount of load

  • Most tendinopathies improve with rehab alone, without injections or surgery

Timeline matters: muscle strength can improve in 6–8 weeks, but tendons often need 12 weeks or more to fully respond — patience and consistency are part of the plan, not a sign it isn't working.

Bottom line: Tendon pain responds to the right load, not less of it. With a structured, progressive plan, most tendinopathies get better — even the ones that have been hanging around for a while.
